Diabetes Medical Management Plan

Safe at School® is pleased to offer its updated Diabetes Medical Management Plan (DMMP) for use by schools and pediatric diabetes health care providers nationwide. This new fillable form will allow for a more standardized and efficient completion of student diabetes care orders through expanded options for prescribers to tailor care instructions to the individual student, providing greater clarity for those charged with the care of children with diabetes in the school setting. Some of the new DMMP’s enhancements include:

  • Comprehensive content that covers the full spectrum of diabetes therapies and approaches, from fixed dosing to novel automated insulin dosing (AID) systems.
  • Specific sections to facilitate the safe and effective use of advanced features of newer diabetes technologies, including exercise settings in devices utilizing AID systems and insulin pump pre-set temporary basal rates that will facilitate improved glucose control; thereby reducing the need to treat low glucose levels and limiting disruption to students’ class and sports participation.
  • The means to provide more precise use of carbohydrates in treating hypoglycemia for those students utilizing diabetes technologies such as continuous glucose monitors (CGM) and AID systems to increase their time in target glucose range.  
  • More comprehensive parent/guardian section (to be completed annually) for families to describe details of their student’s history of care, daily activities, level of independence in self-care, and additional important information to allow for more individualized care in the school setting.  
  • The addition of all-day dosing information for use with overnight field trips and disasters/emergency care when students may be required to be away from home longer than is typical.
  • A condensed and comprehensive one-page medication section that will allow the diabetes care team to communicate interim dosing changes quickly and effectively throughout the school year.

Safe at School® encourages school nurses, diabetes health care providers, and public health staff to become familiar with how to use this form through training and education. 

The DMMP should be updated annually or whenever the child's regimen, level of self-management, or school circumstances change. Parents/guardians are responsible for getting their child’s completed and signed DMMP (or doctor's orders) from their child's diabetes care provider. They should then give the DMMP to their child's school to implement and carry out doctor's orders.
